新興感染症

臨床情報

近年、多くの新しいウイルスやその他の病原体が世界的に拡散していることが観察されています。これらに関連する疾患は「新興感染症」と呼ばれます。この用語は、ある集団で初めて発生した感染症、または発生率や地域分布が急速に増加している感染症を指します。

節足動物によって媒介されるウイルス、いわゆるアルボウイルスは、世界で最も一般的なヒト疾患の原因の一つであり、過去数十年にわたって公衆衛生上の問題として重要性を増しています。アルボウイルスは変異および適応能力が非常に高く、そのためしばしば地域的なアウトブレイク(集団発生)を引き起こします。

デング熱やチクングニヤ熱に加え、2015〜2016年に南米の広範囲で流行したジカ熱も注目を集めました。また、ヨーロッパなどの温帯地域でも、ウエストナイルウイルスやチクングニヤウイルスなどによる季節的なアウトブレイクがこれまでに報告されています。アルボウイルス感染症は臨床症状が非常に類似しているため、臨床像だけに基づいて診断することは困難です。

さらに、SARS-CoV-2のように、世界的な大流行(パンデミック)を引き起こす可能性のある新たな病原体も存在します。これらは公衆衛生上、極めて重大な影響を及ぼす可能性があります。

検査

これらの疾患の診断は、対応するパラメーターに関する商業的な検査システムが十分に整備されていない場合が多く、しばしば困難を伴います。一般的に、新興感染症の診断は、臨床的および疫学的所見に加えて、検査データを総合的に評価して行われます。

検査は、病原体の直接的検出(例:核酸・抗原検査)と、間接的検出(例:特異抗体の検出)を組み合わせて行います。これらの方法は、風土病地域からの帰国者のスクリーニングや、感染症・パンデミック発生時の大規模集団の検査において特に重要です。
